Edits invalidate prior actions
A changed document forces re-approval and re-certification under the same rule as counsel sign-off. Section 16 and Core write to the same record as filings move through counsel and EDGAR.

No. Entries cannot be edited or deleted, and each entry is sealed to the one before it, so any alteration would show.
What binds a sign-off to a document?
The exact version of the full document at the moment of signing. Submission checks that the sign-off still matches the current version.

Yes. Director approvals are recorded against the exact document version and land in the record alongside counsel sign-offs and filing steps.
